Parkinson disease :

Parkinsons disease is a long term neurodegenerative disease that effects mainly the central nervous system affecting the movement , balance and coordination. The symptoms emerge slowly and can be both motor and non motor as the disease progresses. Parkinsons disease is usually sporadic ie., occurs haphazardly from time to time but can have genetic, environmental factors , medications as some risk factors and causes nerve cell death and decrease in the dopamine.

Patients usually present with tremors, rigidity, postural instability, bradykinesia or slow movements, disturbance in mood, behaviour, sleep pattern, cognition ,mask like face ,soft muffled speech, small cramped handwriting and non motor symptoms like constipation, inability to smell, dementia, psycosis, swallowing difficulties, malnutrition, weight loss, aspiration pneumonia. Patients usually have a typical gait with short steps and forward flexed posture. Management of PD is by medications like DRT (Dopamine replacement therapy), dopamine agonists, COMT inhibitors, MAO-B inhibitors, deep brain stimulation, speech therapy, ocuupational therapy and other supportive care.

What are the causes of Parkinson's Disease?

  • Genetic Factors: Although not directly inherited, specific genetic mutations can increase the risk of developing Parkinson's disease. Mutations in SNCA and LRRK2 genes have been associated with an increased risk of Parkinson's.
  • Environmental Factors: Exposure to toxins and chemicals like pesticides, herbicides, and industrial chemicals like solvents has been linked to an increased risk of Parkinson's disease.
  • Alpha-synuclein Accumulation: an accumulation of abnormal protein aggregates, primarily consisting of a protein called alpha-synuclein, in specific brain cells and known as Lewy bodies, are thought to play a role in the degeneration of brain cells.
  • Dopamine Deficiency: a progressive loss of dopamine-producing neurons that help control movement and other functions leads to the motor symptoms associated with Parkinson's.
  • Neuroinflammation: Chronic inflammation in the brain contributes to the progression of Parkinson's disease.
  • Mitochondrial Dysfunction: Dysfunction of the energy-producing structures within cells leads to oxidative stress and damage to cells, including the neurons affected in Parkinson's disease.

What are the symptoms of Parkinson's Disease?

The symptoms of Parkinson's disease vary among individuals, and their severity and progression can also differ. The main motor and non-motor symptoms of Parkinson's disease include:

Motor Symptoms:

  • Tremors: Resting tremors, usually starting in one hand, appear as a rhythmic back-and-forth movement of the fingers or hand.
  • Bradykinesia: slow movement, difficulty initiating and completing movements, reduced facial expressions, shuffling gait, and decreased arm swing while walking.
  • Muscle Rigidity: Muscles may become stiff and resistant to movement, leading to decreased range of motion and difficulty with tasks requiring flexibility.
  • Postural Instability: People with Parkinson's often have difficulty maintaining their balance and posture. This can result in a stooped or forward-leaning posture and an increased risk of falls.

Non-Motor Symptoms:

  • Loss of Smell: In some individuals with Parkinson's disease, there is an impaired sense of smell
  • Sleep Disturbances: sleep problems, including insomnia, restless legs syndrome, and frequent waking during the night.
  • Constipation: due to changes in the autonomic nervous system.
  • Speech and Swallowing Difficulties: The muscles involved in speech and swallowing can be affected, leading to a soft or monotonous voice, slurred speech, and swallowing problems.
  • Depression and Anxiety: Emotional changes like depression and anxiety are common
  • Fatigue: Fatigue and lack of energy are often seen.
  • Musculoskeletal pain: often related to rigidity and altered posture
  • Urinary Problems: such as urinary urgency, frequency, and incontinence
  • Visual Disturbances: such as difficulty judging distances and perceiving contrast.

Different stages of Parkinson's disease are used to identify the progression of the disease. Stage 1 is the initial stage with mild symptoms affecting only one side; in Stage 2, the symptoms are seen on both sides; in Stage 3, more motor symptoms are seen; in Stage 4, the symptoms are very severe; and Stage 5, is the most advanced stage.
Treatment of Parkinson's disease aims to manage symptoms, improve quality of life, and slow the progression of the disease. Treatment options include Medications such as levodopa, dopamine agonists, Deep Brain Stimulation, Physical and Occupational Therapy, Speech Therapy, Exercise and Rehabilitation, Lifestyle Modifications, etc.
Parkinson's disease can have genetic and environmental factors that cause its development.
