Pulmonary hypertension :

Pulmonary hypertension is a condition in which the blood pressure in the pulmonary arteries is elevated thus making it hard for the heart to pump the blood through the lungs ( pulmonary arteries supply blood to the lungs from heart). There is no specific cause but various risk factors include family history , prior pulmonary embolism, sickle cell disease, cocaine use, COPD (Chronic obstructive pulmonary disease), pulmonary fibrosis, heart defects (congenital heart diseases), connective tissue diseases like scleroderma , schistosomiasis, antidepressants, portal hypertension, cirrhosis, chronic thromboembolic diseases etc.

Patient suffering with pulmonary hypertension may present with shortness of breath (dyspnea), fatigue, chest pain, dizziness, swelling in legs and ankles (perepheral edema), increase in heart rate (palpitations), lightheadedness, fainting, bluish tinge to skin and lips (cyanosis), coughing up pink frothy mucus etc. Pulmonary hypertension can be diagnosed by physical examination (to look for any typical signs), Echocardiography (doppler echocardiography), ventilation/perfusion scintigraphy, CT scan etc,. Management includes identifying the cause and treating it accordingly that might might include medical management like vasodilators, CCB's, prostanoids, phosphodiesterase inhibitors, oxygen therapy, lifestyle modifications are required. Surgical interventions like heart-lung transplantation , pulmonary thromboensarterectomy may be done.
